I visit the facility almost every day, and I’ve seen firsthand how every member of the team genuinely cares. Communication with families is excellent—whether it’s regular updates or addressing concerns, the administrator makes sure to keep me informed about my mom’s care, which gives me great peace of mind.

We are incredibly grateful to have found Hazelwood Enhanced Memory Care – it’s more than just a care facility; it’s a community where residents are truly valued. Highly recommended to any family seeking compassionate memory care for their loved one.

I cannot speak highly enough about Hazelwood Enhanced Memory Care. From the moment we first visited, we felt a true sense of warmth, compassion, and professionalism from the entire team. The staff consistently goes above and beyond to ensure residents are treated with dignity, respect, and personalized care.

The facility is always clean, safe, and thoughtfully designed for individuals living with memory challenges. What truly stands out is their commitment to meaningful, personalized activities. I’ve personally witnessed how well-maintained their activity programming is—Ms. Donna, the Life Enrichment Director, starts her mornings engaging the residents with meditation, simple exercises, and other thoughtful activities that truly brighten their day. These small but consistent efforts make a big difference in the lives of the residents.

Initially, my family felt Hazelwood would be a good fit for my mom, but over the past two months of her nearly six month stay, my opinion has declined sharply. While I believe my mother is safe, and her basic needs are being met, I have significant concerns about this facility.

They have fallen short of everything they offered when we toured the facility; activities, 30-60-90 day formal reviews, cleanliness, staffing ratios, etc. There do not seem to be many structured activities - most of the residents are sitting idly or wandering. (Which I know can be normal, but I have had family on my wife's side in memory care and that was not the case.) There have been no formal reviews about my mother's care or situation. The staff is communicative regarding her status, but it's usually along the lines of "she is doing great!".

The cleanliness of the facility has become a problem to me. There is a consistent urine odor and the general areas do not typically feel clean or maintained. There have been Super Bowl decorations that have not been removed for months, and Christmas decorations by the front door since we moved my mother in February 2025.

The staff has constant turnover. I know it is not easy to find qualified care workers, but this place is poorly managed from the top-down. I do not blame the workers, but I cannot tell you how often I observe staff completely checked out and not engaged in the care of the residents. To me, that is a management problem, not a worker problem.

Generally speaking, I would not recommend Hazelwood Advanced Memory Care to anyone seeking placement for their loved one. We are now considering other options. It is my sincere hope, for the sake of the residents, that this place gets its act together, but they need to make some significant changes and hire qualified managers to make that happen.

My grandma was there until she passed this April. The staffed always took such good care of her, helping her with her needs as she became a little more resilient to cares towards the end. The staff would text/call me on my personal cell phone as I was my grandmas POA & lived in a different state. They always kept me informed on what was going on with my grandma. The facility also understood how extremely close my grandma, my mother & myself were & would let my grandma hang on the phone with us sometimes for over an hour!! I have nothing but love and appreciation for this facility and its whole staff! Shout out to everyone! They became family 🖤

My sister been there a week, and enjoys the activities, staff and food. The staff has been awesome caring for my sister who was not treated well at another memory care facility. I tried to take my sister home this weekend she declined I rather stay here with my friends. I cried...My sister said the food is always hot 🔥 and delicious. Thank you Hazelwood staff

I had the opportunity to cut hair here for a client in memory care at this center and the staff greeted me graciously and made me feel welcome. They also made it comfortable to care for this client that I had been cutting hair for before she entered into memory care and we were truly grateful to them for making it possible! Thank you Hazelwood!

Let me start off of saying my Grandmother was in a bad spot. She has dementia, has a hard time walking, communicating what she needs and bathing. Also to mention she is 94, and one of the sweetest ladies Ive ever had the pleasure to have in my life. Her caregiver was no longer able for help her and for two weeks we were trying to get additional help/ care for her. Princess one of the nursing directors at Hazelwood came over on a Friday and saw she needed help. Normally they do placement on the weekend days, but this was an emergency and she was able to place her the next day. The whole process once Princess was involved was so smooth. My Grandmother has been there for 2 weeks and she is so happy, thriving and well taken care of. The facility is clean and the residents are happy. The whole team when you speak to them enjoy being there, joking, and truly is a passion for them. If your loved one needs care Hazelwood is a wonderful place. Our family is there frequently visiting and Im impressed with the professionalism and friendliness of the whole staff. Thank you Hazelwood for taking such amazing care of my Grandmother.

This is the second memory care unit for my wife. I visit daily and have always found her and the other residents clean and well cared for. She tells me and others that she likes it here. I am especially pleased by the way the staff is on top of things when someone needs redirection. All of the staff are helpful, but my go to caregivers are Caesar and Junior. They are always so helpful.
