The staff is super nice, very helpful and extremely knowledgeable! The reason for the one star is because before I was seen we discussed my insurance and financials. Instead of recommending to UOP, they decided it was best that they give me an exam and X-rays and go over a plan we both knew I wasn’t able to afford! So today I paid 75.00 for a very expensive post it note! The system is broken! Yes it’s sucks being me!

I first started as a patient here in Nov 2022.
I was in a lot of discomfort due to TMJ and clicking, and sadly believed everything I was told at the initial consult. I was told what the treatment plan would consist of, the costs associated and that the office would submit bills to my insurance. They made it sound like it is often reimbursed and everything would be fixed in a few months so I went ahead with the $5500 to get started.

The initial appliance took nearly 6 weeks to receive (they told me I’d have it within a few weeks). I was told they couldn’t submit the bill to insurance until I received the appliance. I picked it up early Jan 2023 so I figured my bill was being submitted. I asked about it in mid-February but it hadn’t been sent in. I kept bugging them but they didn’t submit my claim to insurance until late April 2023, and then I learned that insurance will in fact cover nothing. I have very good medical insurance but these services were “performed by a dentist” which the insurance company considers “dental work” and so declined coverage.

I was not given a copy of my treatment plan at my initial visit. When I requested it several months in, all I received was a few hand-written scribbles on a piece of paper. I never spoke with Dr. Samadian at my initial appointment. I have had about 20 appointments at the clinic over the last 2 years and I only saw/spoke to the Dr. one time and that was when he came in to administer the Botox injections. He didn’t even tell me about them; the assistant talked to me and made the (ridiculously high) recommendation of how much I should get.

**If you go in for a consult here get a written copy of the treatment plan upfront. I was told things at that initial appt that changed as time went on. There was an injection they told me was a part of the process but then later, it was not included in the initial price and was quite costly. Also they never explained to me that this was a two-phase process: first devices to realign my jaw (that’s the $5500) and then I had to pay another $7000 for phase 2, wearing aligners to correct my bite. I never knew about phase 2 until 6 months into phase 1. GET EVERYTHING IN WRITING! And check with your insurance BEFORE you agree to proceed if cost is at all a factor. The jaw component is medical and would go through medical insurance. The $7000 in phase 2 was for dental aligners (like Invisalign) and may be covered, at least in part, by your dental insurance. But check before committing to this work.
I was always 5-10 minutes early for my appointments (which were all of 5-10 minutes long as they were usually just quick check ins) and for EVERY appointment, they were always 10-15 minutes late in seeing me. I could listen to other patients concerned about the costs (like I was) and being assured that insurance will be billed, making it sound covered. Be your own advocate and get a written copy of the plan and the cost breakdown upfront. Get a second opinion. And check with your own insurance company.

Personally I would not recommend this office to anyone. I paid a lot of money to Dr Samadian’s office to have only gotten barely 3 minutes of his time once. I don’t even know the title of the employee I’ve been seeing at my visits; maybe a dental assistant? But considering I was told I wore away bone in my joint and needed to work on realigning my jaw and teeth, I would think that would merit a little bit more of the actual dentist’s attention. As I sat waiting for 16 minutes to be seen today, they left the computer screen up and I saw they double-booked. I was given a 30 minute appointment slot at the same time as another TMJ patient. How can that one assistant attend to 2 patients at one time Simple - she cannot. Also, there has been a significant turnover in the staffing at the office from when I started 2 years ago, which says a lot about a place.

Went to this practice for TMJ and masseter Botox after suffering from clenching, clicking, pain and headaches for years. I’ve gone to other practices for the same concerns and my experience here was by far the best. The nurse is SO SWEET — really made me feel heard and safe. Dr. Samadian is also nice. The office was efficient and they never kept me waiting long (which I really really appreciate it in the middle of a workday). Overall 10/10 stop by if you have TMJ!!

Tl;dr Sleep apnea oral appliance works (that earns the 1 star) but I implore you to get one from another office that is trained and knows what they're doing.

I was referred to Dr Samadian for an oral appliance to reposition my jaw and open my obstructed airway at night. I was diagnosed with obstructive sleep apnea years ago and tried everything, including a CPAP for a few years, and eventually a septoplasty and turbinate reduction to correct my deviated septum and open nasal passages. The surgery worked wonders - no more CPAP - but I still had some issues. My dentist suggested an oral appliance as a possible way to address remaining symptoms and kill multiple birds (e.g., grinding) with one stone.

At my first consult I was immediately struck by the hard sell and brusk patient care, including a pay-up-front model, which should have been my biggest red flag. But I went forward and after receiving the oral appliance, began encountering misinformation and inconsistent guidance every time I went in for my spacer adjustments. Every visit I had to reexplain my situation to assistants and sometimes had to correct them based on my treatment history and (because I was growing increasingly concerned) supplemental online research. A few times they even gave me spacers to make the next adjustment at home since the progression and adjustments are fairly straightforward. This was honestly preferable given office appointments were a long wait for a 5-10 minute status check - I once waited nearly two hours because, as far as I can tell, they don't call patients in order and forgot I was waiting in the hallway.

What is especially concerning is that I started noticing lower jaw tension and drift in my bite about halfway through the process. It usually was okay by the end of the day, but I mentioned it in several check-ins and was told it's fine so long as it's not constant or particularly painful. What I've since discovered (going to another orthodontist) is that you're supposed to get a little rubberized bite tab with these oral appliances, which you bite down on hard for 5 minutes every morning to reseat your jaw and jaw muscles. This was never offered or mentioned by Dr Samadian's office, and after getting bite tabs all my jaw issues went away. I don't know for sure what would have happened, but it's scary to think I was on a path of potential long term jaw and bite complications had I not gone for the second opinion. This is incredibly ironic given this office is supposed to specialize in TMJ treatment.

Finally, when I reached the end of the spacer adjustments, Dr Samadian's office wanted to do an at home sleep study to document the improvement with the oral appliance. I was given two machines, one for each night, the fairly standard blood oxygen monitors that clip on a finger. I'm quite familiar with these given my history of sleep apnea and never had a problem before. When I went to bed neither device would start, they were non functional, so I took them back and said I was done - it was such a relief that day to know I would never have to go back to this office.

Again, the oral appliance with the bite tabs works for me. My wife reports minimal snoring, I've done other at home sleep studies since that show I'm doing well, and my consumer grade sleep trackers (ring & watch) support this too. I encourage people to explore it. But it's just a simple dental impression that gets sent off to a 3D printer, with knobs on it for the adjustable spacers. Please make sure you get it through an experienced provider.
